- I understand that massage therapy is provided for stress reduction, relaxation, relief from muscular tension, scar restrictions, and improvement of circulation, lymphatic, and energy flow.
- If I experience pain or discomfort during the session, I will immediately inform my therapist so things can be adjusted to my level of comfort. I will not hold my therapist responsible for any pain or discomfort I experience during or after the session.
- I understand that services and consultations offered are not a substitute for medical care but you as the client are treated as a sovereign, informed adult. I understand that my therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.
- I affirm that I have notified my therapist of all known medical conditions and injuries.
- I agree to inform my therapist of any changes in my health and medical condition. I understand that there shall be no liability on the therapist’s part should I forget to do so.
- I understand that massage is entirely therapeutic and non-sexual in nature, inappropriate behavior, comments or physical innuendos are not tolerated. You will be asked to leave and full payment will be rendered.
- I understand that my health is my sole responsibility, no one else.
- I agree to pay all cancellation and no-show fees should I give less than 72 hours notice of appointment changes without a true emergency. I may also find someone else to take my appointment so I don't accrue cancellation charges.
- I understand social media and late-night texts are not appropriate ways to communicate with my therapist.
- Gift certificates are not redeemable for cash and must be used within 6 months of purchase, the same cancellation policy still applies.
- I understand that the client/therapist relationship may be terminated without notice or explanation if the above policies are not respected.
- By signing this release, I hereby waive and release my therapist from any and all liability; past, present, and future relating to massage therapy and bodywork.
Cancellation fees are as follows:
Due:
50% of your total service for cancellations between 72-24 hours of your appointment time.
Due:
Full payment for cancellations that occur 24 hours or less from the time your appointment is scheduled.
If fees are not paid within 48 hours you will not be able to book future sessions.
